Practice Number Application Forms

  • Kindly ensure that upon submission of your application that all documents are provided as failure to submit all the required documents within a 30-day period will result in the cancellation of your application.
  • The PCNS application fee is non-refundable.
  • PCNS only issues practice numbers for the current period/year. Should you submit an application in the incorrect period/year you will be liable to pay the difference.
