Practice Number Update Forms
All updates to existing practice number information is to be completed on the update
forms below, this includes replacement staff for Ambulance services as well as
Pharmacies.
Please be advised that all banking details updates should be accompanied by a stamped bank letter
not older than 3 months and the relevant Banking Details Update form below. If the update also
applies to the debit order transaction for the renewal of your annual fees, please indicate this on the
debit order form.
Should a bank letter be submitted please complete and submit the PCNS Debit Order
instruction form together with the bank letter.
Should you be updating your dispensing license information, please be advised that you
are to ensure that the physical address on the dispensing license corresponds with the
practice physical address you are updating or the already existing practice physical
address on your practice code number. Kindly submit a copy of the dispensing license.
